IAP : Individual Access Plan

An Individual Access Plan (IAP) provides participants with the opportunity to identify and achieve their goals with one-on-one support. A Community Access Worker (CAW) works with the participants providing support to develop a plan and the steps involved in reaching the participants goals and provides feedback on their progress.

Goals generally consist of the following:

  • Becoming more independent at home
  • Accessing the community
  • Recreational and leisure activities
  • Increasing social and support networks.

The IAP is individually tailored to suit each participant’s individual needs and circumstances.

Examples of IAP goals are:

  • Joining a sporting club e.g. fishing, darts, soccer.
  • Attending TAFE.
  • Budgeting skills.
  • Learning how to use public transport.
  • Improving health and fitness e.g. walking program, visit with dietician, joining a gym.
  • General interest course e.g. learning to play the guitar or dancing lessons.
